YARLY to both. Unless you're being nitpicky; there aren't any upgrades in the final-final level (as far as I can recall), or the extra level (again, pointlessly hiding spoilers), but those are for obvious reasons. Aside from those, every level. And I like that.
I don't think it's even in a properly secret area...It's definitely underwater, though. I hardly ever play as Big once I've done his story, though, so the lure upgrades, though I do always try to get them in each save file I play, don't really make any difference for me.
On the topic of upgrades, I actually really like how there's one in every level of SA2/B.
Right. No wonder you haven't found the fourth; it's hidden inside a level. I found it by accident when I first played the original SA (subsequently granting me the knowledge of where it was for my playthrough of SADX). Is that enough of a hint, or do you want me to tell you which level to look in? I mean, Big only has four levels...
YARLY. And I don't know what the general consensus is about it, but I actually quite like Twinkle Park's hall of mirrors. Which you have yet to discover.
I don't remember what colours they are; I just think of them as '1/4', '2/4', etc.. Where did you find them?
OH and unless I'm mistaken, once you beat Twinkle Park as all the characters that have a full level in it (and I'm saying it like that to prevent spoilers, even though it really doesn't make much of a difference), the characters that don't have a full level in it can get an ID card in Station Square, enabling them to do Twinkle Circuit.
I think the only non-required upgrade for Sonic is the Crystal Ring, which you have...Big has the floatation device (I forget what it's called, but you probably already have it), the rod upgrade that's really badly hidden, and 4 lure upgrades, and I can only remember where 3 of them are...I think. And, yes, Gamma just has those two, as I recall.
And unless my brain has failed me, Tails, Knuckles and Amy all have two each; one required, one optional.
YARLY. Come to think of it, besides the Jet Anklet, there are at least three optional upgrades that are hidden in places I just would not have thought to look ordinarily. One's for Amy, one's for Knuckles and one's for Gamma. I would count Big's rod upgrade, but you actually see where it's 'hidden' in his opening cutscene...
Normally, I'd be all like :-| about skipping Tails' story (for the Sand Hill level, if nothing else), but the gimmick that sets his levels apart from Sonic's sucks. Also, I find that the Jet Anklet upgrade (available immediately, IIRC) is kind of a gamebreaker in some of the levels.

And further analysis supports my idea that the Mario world has suffered an economic crash. So Stars are now worth a heck of a lot less, and NSMB2 revolves around collecting an imperial craptonne of coins, which, clearly, have also suffered devaluation.
THEREFORE NSMB2'S PREMISE IS BASED ON A FAILED ATTEMPT AT QUANTITATIVE EASING.
