A combination of several things, some of which are what you said, her stress-unclear misbehaving (explaining the blood), and she was already getting sick before Hell because she wasn't used to the weather. And I added the detail that their cell is very cold. And yes, part of it is her mental issues. Ellie is claustrophic, though I haven't mentioned that yet.
I'm writing another AU. It's a rewrite of the series, with one difference. Ellie's abuse was much worse, and she ended-up as one of those feral children you see on TV sometimes. I think I PM'ed you something from it once. Anyway, in this fic, Jericho happens to find Ellie (who is called Elena in the story) and takes her to Camp. And he wouldn't be up and leaving every other day like he did, because Elena would have become dependent on him, and he had to think about her.
It kept switching between Jer and Ellie. And there was this creepy thing about ARes telling Ellie that they best way to keep two kingdoms fighting was to take one's princess and frame the other, so he called her Princess the whole time.
